We ask that a physician indicate the need for nursing home services for your loved one. This can be done during a hopitaliztion
or from the physician's office, if your loved one is living at home.
The physician or hospital will send us enough medical information so that we can determine the specitic needs and services
that will be required. From then on we will maintain contact with you, and the healthcare professionals assisting you, until
the admission is complete.
It is important that you and your family member have a smooth transition into our long term care environment. We will
make every effort to that end, keeping contact with you throughout the process.
Then, to assist in your ongoing comfort with Applewood, we invite you to our monthly family meetings to meet and share
with other families. We want you to be free to become as involved as possible in our Applewood life, It makes our home more
HOME for everyone.
